Your Intake Form

To help us prepare for your visit, please take a few minutes to complete your intake form — either online in advance or 15 minutes before your appointment on-site.

IMPORTANT: If you already have diagnostic imaging such as X-ray, CT, or MRI reports, please bring them with you. They help us assess your situation even more accurately.

Please plan around 40 minutes for your visit.

1. Consultation

We discuss your medical history, current complaints, and goals to fully understand your situation.

2. Comprehensive Examination

Depending on your symptoms, this may include chiropractic, orthopedic, neurological, and functional tests. This helps us identify the underlying cause of your complaints precisely.

3. Initial Treatment

We explain the results in detail and then begin with your first treatment.

SECOND APPOINTMENT

Please plan around 40 minutes for your visit.

1. Review of Your Current Status

We discuss your examination results, answer any remaining questions, and explain the underlying causes of your complaints.
We also address key factors such as movement, nutrition, and sleep, and present your individual treatment plan.

2. Treatment & Training

This is followed by a comprehensive chiropractic treatment. We also show you specific exercises for home, so you can actively contribute to your progress.

Please plan around 20 minutes for each follow-up visit.

Continuation of Care

Based on your individual treatment plan, we address restrictions, functional limitations, and muscular imbalances to realign your body and improve function long term.
